GUIDE Dementia Support
Alente Gardens is proud to be the region’s first senior living provider participating as a CMS GUIDE Regional Care Respite Provider, delivering innovative dementia care resources to both residents and families across Southern Illinois.
Our GUIDE program brings Medicare-funded dementia care coordination, caregiver education, and reimbursable respite services directly to families navigating cognitive decline.
Program Components Include:
- Day and short-stay respite fully or partially reimbursed by CMS.
- Caregiver education through our“Are U Ready Now” faith-based community outreach initiative.
- Access to RPM and CCM services for ongoing clinical support.
- Dedicated team including RN oversight, respite aides, activity professionals, and community outreach coordination.
With projected annual GUIDE program revenue exceeding $1.1 million by Year 3, Alente Gardens is uniquely positioned to sustain high-quality, accessible dementia care while reducing caregiver burnout.
